SPECIALIST SUPPORT COORDINATION

SUPPORT FOR COMPLEX NDIS NEEDS

Complex Connect provides Specialist Support Coordination for NDIS participants with complex support needs and circumstances. We help participants, families and support teams navigate barriers, coordinate multiple services and build a more stable and effective support environment.

Our approach is particularly suited to situations where disability intersects with other systems or challenges, including housing, hospitals, mental health services, justice involvement, behaviour support, SDA and multiple service providers.

We focus on understanding the whole situation — not treating each issue in isolation.

What is Specialist Support Coordination?

Specialist Support Coordination is a higher level of support coordination for NDIS participants whose circumstances involve significant complexity or barriers to implementing their NDIS plan.

A Specialist Support Coordinator can help identify barriers affecting a participant's supports, coordinate services and providers, assist with complex service arrangements and work with the participant and their support network to build greater stability and capacity over time.

When might Specialist Support Coordination be helpful?

Specialist Support Coordination may be helpful when a participant's disability support needs intersect with multiple services, systems or significant life circumstances. This can include:

• Justice system involvement
• Housing instability or risk of homelessness
• Complex hospital or prison discharge
• Mental health and psychosocial complexity
• Alcohol and other drug concerns
• SDA applications or complex housing arrangements
• SIL and supported living arrangements
• Behaviour support needs
• Multiple providers or services that require coordination
• Significant barriers to implementing an NDIS plan

Help Navigating NDIS Funding and Service-System Changes

NDIS funding decisions, plan changes and changes to the way particular supports can be accessed can create significant challenges for participants with complex needs.

Complex Connect helps participants and their support networks understand how changes may affect their supports, identify practical options and coordinate services where existing arrangements are no longer working effectively.

Our broader experience across disability advocacy, legal and complex service systems also helps us recognise when an issue extends beyond support coordination and involves other systems or processes.

Specialist Support for Situations That Aren't Straightforward

Complex situations rarely involve just one problem. A participant may be dealing with disability support needs alongside housing issues, hospital discharge, justice involvement, behaviour support, family pressures or several providers that aren't working effectively together.

Complex Connect's approach is to understand how those pieces interact and help bring the right people, services and supports together.

Our team combines practical NDIS knowledge with experience across disability, behaviour support, advocacy, legal and complex service systems. This broader perspective can be particularly valuable when standard approaches to support coordination haven't been enough.
