Complex Connect provides Coordination of Supports (Level 2) for NDIS participants who want help understanding their plan, connecting with suitable services and building a support network that works for them.
Our Support Coordinators work alongside participants, families and support networks to put supports in place, address barriers and build greater confidence in managing NDIS supports over time.
We take the time to understand the whole situation — your goals, your support network and any other circumstances affecting how well your supports are working.
Coordination of Supports is a Level 2 support coordination service that helps NDIS participants understand and use their plan, connect with providers and other services, and build the skills and confidence to coordinate their supports.
Your Support Coordinator works with you to establish a mix of supports that reflects your goals, needs and circumstances. This may include NDIS providers as well as community, mainstream and other government services.
The aim is not simply to organise services for you. It is to help you build the knowledge, confidence and capacity to have greater choice and control over your supports.
Depending on your circumstances, your Support Coordinator can help you:
Having an NDIS plan doesn't automatically mean that finding and coordinating the right services will be straightforward.
You may need to compare providers, establish new services, coordinate several organisations or work out why existing supports aren't achieving what you expected.
Complex Connect can help you look at how your supports fit together, identify gaps and explore practical options that align with your goals and available funding.
Where several providers are involved, we can also help improve communication and coordination so that your supports work together more effectively.
Good Coordination of Supports should do more than manage services on your behalf.
A key part of the role is helping you build the skills and confidence to understand your plan, make informed decisions, communicate with providers and have greater control over your supports.
How this looks will be different for every participant. We work at your pace and focus on the areas where support will be most useful, while respecting your preferences and decisions.
Sometimes the difficulty isn't simply finding a provider.
Housing, family circumstances, mental health services, behaviour support, hospital discharge, justice involvement or several service systems may all affect how an NDIS plan can be implemented.
Complex Connect's broader experience across disability, advocacy, legal and complex service systems helps us understand how these different parts of a person's circumstances can interact.
We can help identify barriers, coordinate the people and services involved and work with you to find a practical way forward.
You don't need to have everything figured out before contacting us.
If you have support coordination funding in your NDIS plan and aren't sure what support you need, contact Complex Connect to discuss your circumstances, your goals and what is currently happening with your supports.
We can talk through the next steps and whether Complex Connect may be a suitable provider for you.
