Specialist Services
Specialist Case Management
Specialist case management and coordination for people navigating complex disability, rehabilitation, mental health, housing and justice systems. Bringing services together, addressing barriers and finding practical ways forward.
When someone's support needs span disability, health, housing or justice systems, knowing how to move forward isn't always straightforward. Complex Connect brings extensive experience working with people whose circumstances involve acquired brain injury, complex psychosocial disability, forensic or justice-system involvement, and significant difficulties accessing or maintaining appropriate supports. Our leadership team brings together backgrounds in psychology, acquired brain injury services, TAC rehabilitation programs, legal practice and Specialist Support Coordination. This experience helps us understand how different systems interact and the practical challenges that can arise when several organisations are involved. We welcome enquiries about specialist case management, complex coordination and consultation, with each referral considered according to the person's circumstances, our capacity and the relevant funding arrangements.
What Specialist Case Management May Include
- Coordination of identified needs, risks and service planning across complex circumstances
- Multi-agency coordination across disability, health, mental health, housing and justice systems
- Transition planning and support (hospital to community, justice to community, residential transitions)
- Consultation to support teams, families and other professionals
- Coordination of identified safety considerations and service planning in complex situations
- Support during periods of significant change or instability
- Liaison with legal, justice, health and government systems
Who May Benefit
We welcome enquiries from individuals, families, hospital and rehabilitation teams, allied health professionals, legal representatives, insurers and other professionals working with people who have complex support needs. Specialist case management may be particularly relevant when someone is preparing to leave hospital following an acquired brain injury, transitioning from an institutional setting into the community, experiencing difficulties maintaining suitable accommodation, or requiring coordinated involvement from disability, mental health, health and other services. We can also assist with initial enquiries where several organisations are involved but responsibilities are unclear, existing arrangements aren't working, or a family or professional needs guidance about the appropriate next steps. Enquiries may involve private funding, TAC, WorkSafe or other funding arrangements. Each enquiry is considered individually, with service delivery subject to suitability and any necessary funding and provider approvals.
Enquiries may involve NDIS, TAC, WorkSafe, private or other funding pathways. Each enquiry is considered individually.
